Insurance, Payment and Billing Information

We make every effort to work with each patient’s insurance company.

We are here to assist you with insurance questions, pre-certifications of surgeries and tests and any other inquiries that you may have.

If you have insurance that requires a special referral from your personal physician, please call their office to ensure the referral has been done.

If your insurance company requires a co-payment, you will be asked to pay that the day of your appointment.

Please bring your insurance cards to your appointment.

All workers’ compensation cases must have a letter of authorization from your worker’s compensation insurance company before we can see you.
