BENEFIT DESCRIPTION ELIGIBILITY
Earned Time Off Hospices's time off program (The accrual consists of 7 holidays, 5 sick days, and 10,15, or 18 vacation days depending on years of service.) Accrual is based on employment status and years of service. Full-time (64 hr/pp) employees with 1-4 years of service earn 22 days (176 hrs) annually. Employees working 40-80 hours/pay period. ETO begins accruing immediately and may be used after 3 months of employment if performance is satisfactory.
Extended Illness Bank Hospice's short-term disability bank for employees in case of personal illness. Accrual is based on employment status. Employees can use EIB after being absent from work due to personal illness or injury for 3 consecutive working days, or 24 consecutive scheduled hours. Employees working 40 hours a pay period accrue 5 days (40 hours) a year. Full-time employees (64-80 hours/pp). EIB begins accruing immediately and may be used after 3 months of employment if performance is satisfactory.
Medical / Dental Insurance Health: Physician's office visit co-pay is $20 per visit, then 100% benefit. for other services, individuals have a yearly $1,000 deductible, then an 80/20 co-pay with a minimum of $3,000; families have $1,000 deductible, then an 80/20 co-pay with a maximum of $6,000. Lower deductibles are available.
Dental: 80/20 co-pay. Annual deductible is $25 per individual or $75 per family. Diagnostic, preventive and ancillary services are not subject to deductible.

Full-time employees (64-80 hrs/pp).
Eligible to enroll the first month following 30 days of employment. Hospice subsidizes a portion of the premium for employees.

Life / Accident, Death and Dismemberment Insurance Coverage is 100% of annual salary rounded up to the next highest thousand. There are additional provisions for accidental death and dismemberment. Employees working 40-80 hrs/pp covered free of charge. Effective first of the month following 30 days of employment.
Retirement To qualifying participants, Hospice makes an annual contribution for each of the employee's credited years of services, (1-9 years of service, 2% hospice contribution and 9+ years of service, 2.5% contribution). Employees may voluntarily contribute to the plan as well and Hospice will match 50% up to 3% or 3.15% depending on credited years of service. 21 years of age; one year of employment; work at least 1,000 hours.
Enrollment is January 1 or July 1 of each year.
Education Assistance Hospice provides education assistance for conferences, workshops and continuing education. Employees may request coverage of expenses related to education programs. when employee attendance at an education program is required, the employee will be compensated for time spent in attendance, as well as registration and travel costs. Tuition reimbursement may be provided for employees who meet criteria. All employees
Travel Reimbursement Employees are reimbursed the IRS standard mileage allowance for use of their vehicles for job related activities. All employees
Bereavement Leave Hospice provides paid leave of up to 3 days for a death in the family to include spouse, parent, parent-in-law, grandparent, grandparent-in-law, children, and step-children. All employees
